The need for care, care and therapy is growing and the workload in the health professions, especially the nurses, increases. At the same time, interprofessional exchange and the documentation have become much more and more time -consuming.
The aim of the almost five -year Linked Care Research project was to make interprofessional cooperation easier using a digital process for medication order. Special focus is on communication between care and support, general practitioners and pharmacies. The Campus Wien University of Applied Sciences has worked intensively together in an interdisciplinary project consortium with a total of 13 partners from mobile care and care organizations, software companies and science partners in order to develop and test a practical digital solution.
Linked Care creates a safe flow of information in mobile care/support, so that all those involved in the health professions work efficiently with a strong consideration of the client and relatives. Cooperation with other stakeholders such as the professional associations, the political responsible and the ELGA was decisive for success.
